As part of the Terms and Conditions of accepting a Daiwa Adrian Prize, recipients must submit a report to the Foundation within two months of completing the project. 

Recipients MUST use the Foundation's report form. The form is a PDF file and should be viewed using Adobe Acrobat Reader. If you do not have Acrobat Reader, it can be downloaded for free from Adobe.

Please complete the form on-screen. When it is finished, print it out, sign it and return to us by post. Reports sent by email or fax will NOT be accepted.
